Myobloc™ :: Dilution & Storage
Dilution
Myobloc™ can be used undiluted straight from the bottle as is done by Dr. Baumann, or it can be diluted to achieve the desired concentration per 0.1cc of solution. For example, if the physician desires to inject 0.1cc of solution that contains 500 units of Myobloc™ per 0.1cc, no dilution would be necessary. If the physician wants to inject of 155 Myobloc™ units per 0.1 cc of solution, a 2500 unit vial of Myobloc™ can be diluted in the original bottle with 0.5 cc of normal saline.
Because each 0.5 cc bottle of Myobloc™ containing 2500 units is overfilled to 0.82 cc, this dilution results in a concentration of 155 Myobloc™ units per 0.1 cc. See the following table for injection guidelines. (Remember that because the vials are over filled, it is important to dilute differently in the syringe than one would dilute directly in the bottle.) This seems very complicated, however no dilution of Myobloc™ is necessary. Dr. Baumann prefers to use Myobloc™ directly from the vial and injects 500 units of Myobloc™ (0.1cc) per injection site as discussed in the technique section.

Storage
Myobloc™ is packaged in a prediluted vial that is stored in refrigeration. Three vial sizes are available: 2500 units, 5,000 units and 10,000 units. These vials can be further diluted, if desired. The varying sizes that are available are nice because a fresh vial can be opened for each patient. If one injects as described by Dr. Baumann, it will require 1500 units to 2500 units to treat the glabellar area; therefore one 2500 unit vial of Myobloc™ could be utilized for this purpose. The crow's feet require 2500- 3000 units and the forehead requires 2000 - 4000 units. Myobloc™ vials contain the exact same dilution regardless of how many units they contain. For example, a 2500 unit vial contains 0.5 cc of solution, while a 10,000 unit vial contains 2cc of solution. Therefore, the remnants of one vial can be easily mixed with the remnants of a leftover vial without problem. In addition, opened vials remain stable for long periods of time. Remember that the vials should not be frozen as this could lead to a loss of potency of the toxin.
